Complex Ovarian Mass – Endometrioma or Functional Cyst?
Hi,
I was hoping that someone could provide me with some advice. Below are my reports, this has been going on for more than 10 months. I saw one Gynecological Oncologist and she said that the reports show functional cysts (despite having them for 10 months) that will go away by themselve and no treatment is needed. That was last week.
I saw another Gynecological Oncologist this morning for a second opinion and she said that they are Endometrioma. She has scheduled an Trans-vaginal ultrasound for Jamuary 21, and I will get those results from her on February 4, 2009.
I am completely confused. I do not know which one to trust. Worse still – will these continue to grow? What if they are neither? My CA125 was 63. My father died of colon cancer and my paternal Aunt of Breast Cancer.
My first report dated February 8, 2008 states:
Endometrium double layer thickness is just over 7mm.
The uterus is anteverted with overall diameters 37x40x76mms. There
appears to be a fibroid subserosal posterior at the fundus 24x19x14mms.
In the cervix, there is a little cyst 8mms.
The right ovary is 34x28x22mmx. Here we find 2 follicles or a
biloculted cyst with a thick septum, more likely that these represent 2
adjacent follicles.
The septation between the two are little hyper vascular.
The left ovary is 30x30x28mms contains a follicle 13x8mms.
The cul-de-sac is clear and the bladder appear normal.
Report 2 dated Sept 22, 2008:
The uterus measures 7.7×4.8×3.7cm. There is a fibroid at the fundus
measuring 2.2cm. Endometrial lining is 0.4cm. In both
adnexa there are abnormalities. On the left there is a complex
primarily solid but inhomogeneous irregular mass measuring
6.1×4.1×3.7cm. On the right there is one measuring 2.5×2.4×2.3cm.
Sorry, should have mentioned that I am 37 years old, have never had kids, and got my period when I was 13. Symptoms: bloating pelvic tenderness, lower back and leg pain, headaches, constant need to urinate (sometimes a lot, sometimes a false call!) and extremely tired. Very heavy and painful menses.
Hi. Sorry to hear about all this trouble you’ve been having. I had ovarian cancer some years ago. Of course I can’t say what exactly is wrong with you, if two gyn oncs who examined you are both unsure what you have. So I can’t make any judgment one way or the other. But if I were you, I’d schedule an appointment with another gyn onc for a third opinion. You can bring your scans. It sounds like the second gyn onc isn’t too worried, or she’d have scheduled further testing immediately rather than in a month.
Your symptoms could mean a variety of things–that’s one of the difficulties of ovarian cancer. Its symptoms are also the symptoms of many common and not terribly dangerous ovarian problems. Likewise your CA-125. This test, while used for detection of ovarian cancer, is actually testing something that rises in your body in the presence of any abdominal inflammation. So the things you currently have–a cyst, maybe uterine fibroids–could cause a rise in CA-125 all on their own, with no need for them to be cancerous.
can ovarian cyst cause you not to get pregnet?
i been having cyst since 2005, this month i found out i have 1 again but on my right side, its a complex cyst measuring about 3×3.6×4.7cm with debris and also septation, i had missed my period for about a month and last night i had sex and i started bleeding, so i got my period and its very dark with some clear stuff with it. has anyone out there had this before and can tell me a lil about this cause this tI’me im a lil scared,
i know im a guy but my good friend at work was missing her periods and having irregular ones for months with consant bleeding. it turned out an embryo was developing in her ovary. You should go to your ob/gyn.
does ovarian cyst can cure by surgery only?
I was enjoying healthy life but since 5th november my periods were not getting stoped so i counsult to gynaecologist she has suggested for ultrasound there is evidence of a large measures 12.7*7.1*7.2 cms(volume-341 cc.) complex,predominantly cystic elongated mass lesion with septation seen in right adnexa extending to pouch of Douglas.
FINAL IMPRESSION
LARGE COMPLEX CYSTIC RIGHT ADNEXAL MASS EXTENDING TO POUCH OF DOUGLAS?TUBO-VAGINAL IN ORIGIN
….now can any body explain me about this finding and treatment as well….??…can surgery avoided…right now I have pain in abdomen .my periods are stopped
can this much big cyst cured by surgery??or they will remove ovary as well??..i am scared…pls ans…I have minor
pain in abdomen which increase if I increase physical activity.
I don’t know enough to say if your cysts are more dangerous than mine but my periods weren’t stopped so I’d guess they are. So I’m not sure that this would work in your case but my doctor suggested that I begin to eat more carefully and will hopefully starve my cysts. She said that mine were fed by the excess sugar in my blood and that if I began eating as if I were diabetic I’d probably be able to get rid of the cysts.
complex ovarian cyst?
I went to the ER yesterday because I had a sudden pain in my stomach that didn’t ease up. They did an internal ultrasound and found that I have multiple cysts on my left ovary, one measuring 2in. by 1in. that demonstrates some heterogeneity with internal septation and complex echoes. I also have multiple cysts on my right ovary that they described the pattern as being unremarkable. I’m not sure what all this means and it kind of sounds scary. I have an appointment with a gyn. on Friday. I had asked the ER doctors a few questions but they didn’t seem to want to answer any of them and just wanted to rush me out. I have had similar pains for nearly 2 years before now and had went to the ER and my gyn. when they first began, but it was written off as ovulation pain. Could this have somehow been treated before so many cysts developed and gotten so large? I am also wondering if this could be the reason I have been unable to concieve and if it will prevent it in the future?
If your doctor couldn’t tell you I would go to a different doctor, probably a gynicologist. I think that your cysts could have been avoided if they were caught earlier. If you have cysts on both ovaries then this is probably why you couldn’t concieve and they might have to remover your ovaries to remove the cysts so you might not be able to concieve either.

I had another ultrasound on November 6 – I do not have the results. I am due to see my Gynecologist on December 16. For now she says suggests that we do another ultrasound in 6 weeks – it has already been 10 months and we know that the mass is growing. Do the above reports suggest anything that I should be concerned about?
I am 37 years old (28 on January 10). I have not had any children. Last week I found out that my FSH levels are 15. My father died of gastrointestinal cancer, my mother kidney cancer, and my paternal Aunt had breast and kidnehy cancer.
Thank-you for your answers. To be honest I have not been going for yearly check-ups – in fact the one I had in Feb. 2008 was my first in over 8 years, I despise going. What is frusturating is that I keep getting told to wait, and meanwhile it continues to grow!
i wouldn’t WORRY as such, but yes this is not normal.
a cyst is common, it does not have to be cancerous and if it is problematic it will be removed.
It sounds like you might have some cancer. if you have been having regular yearly checks then hopefully you have caught it in the early stages which means that it is not a big deal at all. One in 8 woman get cervical cancer.
